Coordinated individual plan (SIP) and collaboration

Municipalities, regions, private providers and authorities are currently facing major challenges in recruiting staff for health and social care. An equally great challenge is the reduction in financial resources. With increasing demographic demands and decreasing welfare resources, collaboration needs to be streamlined.

In order to make more efficient use of welfare resources, health, healthcare and social care need to be provided with the right interventions, by the right organization, at the right level and at the right time. This is a prerequisite for managing the health, care and welfare of the future, where children and young people need to be able to succeed at school, adults need to have the conditions for increased work participation and the elderly need to be able to age in good health.

We solve this streamlining by offering business development and a changed way of working! The goal is to contribute to more person-centered care, a competitive work environment and coherent health and care chains.

Training/lecture

We offer training in SIP, collaboration and person-centered approach, both physically and digitally. The offer is aimed at everyone who works with health, care and care in both the municipality and the region, whether it concerns children and young people, adults or the elderly. Other partners are also offered SIP training, such as the Swedish Social Insurance Agency, the Swedish Public Employment Service and the Prison and Probation Service.

Consulting activities

We offer consultants for short or long term assignments.

Our consultants have work experience from several business areas and experience in leading change processes in large organizations.
Our consultants are outgoing, pedagogical, meticulous and structured and take their own initiative. Of course, they have experience of working with both internal and external contacts. This has led to the development of good cooperation skills, express themselves well in both speech and writing, and are flexible and service-oriented. Our consultants work just as well independently as in groups. Naturally, the consultants have knowledge and experience of change and process management methods.

For successful collaboration - it doesn't happen by itself!
